چکیده

Because of the obligate intravascular location of contemporary continuous-flow ventricular assist devices (CFVADs), it is not surprising that bleeding and thrombosis are among the most common and feared complications of this therapy, respectively. Although strides have been made in our understanding of the pathophysiology of continuous flow, intravascular CF-VADs have been widely used only in the last few years, and a great deal remains less well known (or unknown). Outcomes have improved significantly over the past decade, but a deeper understanding of how these devices perturb the delicate intrinsic balance of bleeding and thrombosis is critical to guide therapy and to overcome the current obstacles to achieving a long-term safety profile comparable to that of mechanical prosthetic valves.

Complications of Continuous-Flow Mechanical Circulatory Support Devices

Left ventricular assist devices (LVADs), more importantly the continuous-flow subclass, have revolutionized the medical field by improving New York Heart Association (NYHA) functional class status, quality of life, and survival rates in patients with advanced systolic heart failure. From the first pulsatile device to modern day continuous-flow devices, LVADs have continued to improve, but they ...

Watchful Waiting in Continuous-Flow Left Ventricular Assist Device Patients With Ongoing Hemolysis Is Associated With an Increased Risk for Cerebrovascular Accident or Death

Continuous-flow left ventricular assist devices (CF-LVADs) have led to a significant improvement in the survival of patients with end stage heart failure. However, CF-LVAD use is complicated by numerous serious adverse events, with bleeding and thrombosis accounting for 51.3% of readmissions. Device thrombosis in particular is a feared complication of long-term CF-LVAD support, occurring in ≈10...

the higher risk of developing a thrombosis anyway. In conclusion, compared with the other study results, our data suggest that a notably mild anticoagulation protocol does not increase the risk of severe thrombo-embolic events in the early and late postoperative period after the implantation of the HM II CW LVAD. Furthermore, this mild regime is helpful in reducing the still unacceptably high i...

Relationship Between Anticoagulation Intensity and Thrombotic or Bleeding Outcomes Among Outpatients With Continuous-Flow Left Ventricular Assist Devices.

BACKGROUND We evaluated thrombotic and bleeding outcomes in patients with continuous-flow left ventricular assist devices (CF-LVADs), stratified by anticoagulation intensity. Previous studies of outpatients with CF-LVADs have suggested that target international normalized ratio (INR) values <2.5 (range, 2-3) may be used. However, recent studies reported an increase in pump thrombosis among CF-L...
